By participating in any programs or activities at Elite Mindset Training Facility / Elite Mindset University, including but not limited to basketball training, open gym, practices, games, drills, strength and conditioning, agility workouts, leagues, camps, clinics, group sessions, private training, and all facility-related events, I acknowledge that:
I voluntarily choose to participate or allow my child to participate and accept full responsibility for all risks, known and unknown.
I release and discharge Elite Mindset Training Facility, its owners, trainers, employees, staff, contractors, and affiliates from all liability, claims, demands, or actions related to:
This release applies before, during, and after activities.
In an emergency, I authorize Elite Mindset Training Facility staff to obtain necessary medical treatment. I agree to be financially responsible for all medical costs. I confirm that the participant is physically able to safely participate.
I understand and agree that Elite Mindset Training Facility may photograph, video record, and use images or recordings of the participant for marketing, social media, advertisements, website content, and training material. I grant full permission without compensation.
To participate at Elite Mindset Training Facility, all athletes and parents must follow the rules below:

Payments secure a training spot and are non-refundable. In rare cases of medical emergencies, credit may be issued at management discretion.
